APPOINTMENTS

• New patients will arrive 15 minutes before their appointment to complete paperwork.
• As a courtesy to our patients, we will call to confirm your appointment the day before. If we leave you a message, please call our office to confirm.
• Our appointment time is valuable, therefore we require 24 hours notice in the event of a cancellation of a previously scheduled appointment. The office policy is that a patient and their siblings may be inactivated from the practice should there be two appointment "no shows" or cancellations with less than 24 hours notice.

PAYMENTS

**Payment is due in full at the time of service* *

We accept the following:
• Check (only accepted if the person writing the check is listed; no temporary checks; no third party checks)
• Cash
• Mastercard or Visa only (We do not take Discover or American Express)
• CareCredit

It should be noted that personal checks returned because of insufficient funds are subject to a $35.00 late fee. Full payment plus the $35.00 fee is due within 5 business days from the time our bank notifies us of a returned check. Failure to make payment will result in the check writer's name and personal information being forwarded to the Harris County District Attorney's office for full prosecution under the laws of Texas.

TREATMENT

• We strongly recommend that you do not bring other children with you to the appointment because you need to give your child your undivided attention.
• Parents are invited to come to the back with their child for exams and cleanings, but will have to wait in the waiting room during any operative procedure.
• Please let us know in advance if your child has a fever or any contagious condition, including the flu, so we can reschedule his or her appointment.
